Develop, coordinate, and implement outreach programs aimed at educating communities about health services provided by Upward Health. Build and maintain positive relationships with community leaders, organizations, and health care providers to facilitate collaborative projects. Organize and participate in community events and presentations to increase awareness and participation in Upward Health programs. Analyze community needs and feedback to tailor outreach efforts and improve service delivery. Prepare detailed reports on community outreach activities and their impact on patient engagement and health outcomes. Assist with the creation of marketing materials and social media content to enhance visibility and community engagement.
