We are seeking a motivated Community Outreach Specialist who will be instrumental in promoting our services and enhancing patient engagement through effective community interaction and educational outreach. This role requires a compassionate individual who can adeptly coordinate with healthcare professionals to deliver exceptional care tailored to our patients' unique needs, right in their homes. Develop and implement outreach programs to educate and engage the community about Upward Health?s services. Build strong relationships with local organizations, patient advocacy groups, and healthcare providers to facilitate comprehensive care coordination. Organize and lead community events, workshops and presentations to raise awareness and promote the benefits of home-based healthcare services. Collaborate with healthcare providers, including clinicians and social workers, to identify patient needs and direct them to appropriate services. Track and analyze community outreach efforts and adjust strategies as needed to improve outcomes and extend reach. Educate potential patients and families about the services provided by Upward Health, ensuring they understand how to access these services. Participate in continuous education to remain knowledgeable about trends and developments in home healthcare.
