Upward Health is seeking a dedicated Community Outreach Specialist to join our dynamic team, dedicated to providing high-quality, in-home care for complex patients. The successful candidate will be instrumental in developing community relationships and enhancing patient outreach. This role involves strategic collaboration with local healthcare providers, community organizations, and support groups to facilitate comprehensive care tailored to individual patient needs. Develop, coordinate, and implement health outreach programs and initiatives within the community to promote the services of Upward Health. Build and maintain partnerships with local healthcare providers, community leaders, and other stakeholders to facilitate integrated care. Organize and participate in community events, workshops, and health screenings to enhance community engagement and service uptake. Conduct outreach efforts to identify and engage patients who can benefit from Upward Health?s services, particularly those with complex health needs. Work closely with healthcare professionals and care coordinators to ensure seamless service delivery and patient care continuity. Prepare reports on community outreach activities and outcomes to inform strategic planning and project funding. Educate community members about health issues, available services, and eligibility criteria for various health programs. Strong interpersonal and communication skills. Ability to work independently and as part of a team. Experience with community engagement and public health education. Strong organizational and project management skills.
