**Job Description:**

We are seeking a dedicated Home Health Registered Nurse to provide compassionate care to patients in their homes. This role requires expertise in home health and proficiency in HCHB/OASIS documentation.

**Key Responsibilities:**

• Deliver quality nursing care to home health patients.
• Complete HCHB/OASIS documentation at patient visits.
• Manage a caseload of 20•25 cases weekly.
• Attend weekly in•person office meetings.
• Be flexible to float as needed.

**Requirements:**

• Active RN License and BLS Certification.
• 2+ years of home health experience.
• Proficient in HCHB/OASIS, including Oasis E.
• Reside within 30 minutes of the territory or secure nearby housing.

**Contract Details:**

• 13•week contract, 40 hours/week.
• No overtime or on•call duties.
• Pre•approved time off must be made up.

**Additional Information:**

• 2•3 days of orientation.
