We are seeking an experienced and compassionateCare Delivery Manager to join our interdisciplinary team in Flushing, NY,supporting a high-quality PACE (Program of All-Inclusive Care for the Geriatrics)care model. In this leadership role, you will oversee daily clinicaloperations, ensure coordination of services, and support the delivery ofculturally competent, person-centered care to our diverse geriatric population.
Compensation:
$55 - $64.50 Per Hour
Schedule:
Monday-Friday 8:30 am-5:00 pm
Key Responsibilities:
Oversee the development, execution, and continuous improvement of individualized care plans that promote satisfaction, safety, and health outcomes Facilitate and participate in Interdisciplinary Team (IDT) meetings, including Service Delivery Requests (SDRs), and complete necessary documentation and attestations Serve as a manager on-call during off-hours to ensure continuity of care and address urgent participant or caregiver concerns Generate and analyze Clinical Delivery reports to support care coordination and operational decision-making Represent Community Health Nurses (CHNs) during IDT meetings and provide clinical leadership and oversight as needed Monitor, balance, and allocate staffing resources across the site to maintain seamless service delivery and adequate team coverage Maintain consistent communication with all team members and external partners to ensure care aligns with the participant's goals, needs, and preferences Schedule:
Monday through Friday 8:30 AM - 5:00 PM Location: PACE site in Flushing, NY Position is Temp to Potential Perm Qualifications:
Korean Bilingual preferred Degree from an NLN-accredited nursing school - RNor BSN required Bachelor's degree strongly preferred Successful completion of NYSED-approvedinfection control coursework Minimum of 8 continuing education contact hoursincluding mandatory training requirements Case Management Certification recommended within2 years of employment Current active and unrestricted license andregistration in New York State as a Registered Nurse is required 2+ years of administrative experience in amanagement role in CHHA, LHCSA, acute care, nursing home, or diagnostic andtreatment clinics Supervisory experience required Experience in managed long-term care required At least 1 year of experience with frail orelderly populations preferred (PACE training provided if less) Strong customer service and interpersonalcommunication skills Proven ability to work across disciplines andmanage complex care environments
