Discharge Nurse
Our Discharge Nurse works with our medical team, clinical team, and nursing team to facilitate the discharge process for our patients. This position works one on one with our patients and is an important part of our patient's care and treatment. Our Discharge Nurse ensures our discharging patients understand their medications, reviews their discharge instructions, and reviews all post-discharge scheduled appointments.
Essential Job Duties:
- Facilitate discharge process with patients
- Ensures all discharging patients receive discharge paperwork
- Review discharge paperwork with the patient
- Ensure patients discharges with necessary medications
- 3 Days per week, Every other Weekend Mandatory. 12 hours per day.
Education/Experience/Skill Requirements:
- Graduation from an accredited school of nursing required.
- One or more years' nursing experience within clinical area of service provided by facility preferred.
Licenses/Designations/Certifications:
- Current RN license as required by state.
- CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
