We are seeking a bilingual candidate with a bachelor's degree and proficiency in Microsoft tools who is flexible to work weekends, be on call, and handle late shifts! Purpose & Impact: The Youth Case Management Specialist will manage service plans and assessments required to create comprehensive case files. This role demands a flexible and organized approach and may involve extended work hours, weekends, and on-call duties. Key Responsibilities: Conduct initial intake interviews with youth, gathering family and sponsorship information while confirming the youth's age. Interview family members, friends, and sponsors to assess relationships and validate information within 24-48 hours of youth's shelter admission. Evaluate options for youth within 48-72 hours and proceed with documentation for reunification with family, either in their home country or in the U.S. as applicable. Ensure timely completion of all assessments like Initial Intake, Emergency Placement, and Preliminary Service Plans following LSF, State, and Federal guidelines. Document all interactions with youth, sponsors, and stakeholders through progress notes as required. Prepare and submit reunification packets for review, ensuring timely release of youth to designated representatives. Provide weekly updates to youth and telephonic updates to their families or sponsors. Assure two weekly contacts with the youth's family and caregivers, and facilitate calls between minors and approved family members. Coordinate weekly meetings with treatment team representatives from all shelter departments. Maintain both physical and digital client files efficiently. Drive clients to necessary appointments, including court and attorney visits, as required by contractual obligations. Additional Duties: Participate in mandatory training and departmental meetings to stay updated on policies and practices. Engage in training for professional development in case management and documentation. Travel as needed for training or to transport youth. Ensure a safe and clean working environment. Supervise youth adequately at all times. Adapt to changes constructively and manage other essential tasks as assigned. Qualifications: Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or related field. Experience: Preferred 1 year of experience working with adolescents or in the youth services field (volunteer and internship experience included). Skills: Proficient in Microsoft Windows applications (Word, Excel). Valid state driver's license required for transportation duties. Flexible schedule to meet program service needs.
