Specialty: Case Management Discipline: RN Start Date: 08/04/2025 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel POSITION SUMMARY: The Hospice Nurse Case Manager admits patients to service, assesses the patient’s and family’s physical, psychosocial, environmental, safety, and developmental needs. They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment and performance improvement program, and conducts follow-up visits as needed. 1 Year of Hospice Nursing Experience Required Territory Details: Atherton, Menlo Park, Palo Alto, Mountain View, Sunnyvale, Santa Clara, San Jose, and neighboring cities as needed.
