**Job Title: Home Health Physical Therapist**

Location: Kenosha, WI

**Position Overview:**

• Contract: 13 weeks, potential extension to 26 weeks
• Schedule: 32 hours/week, Monday•Friday, 8:00 AM • 4:30 PM
• No weekends or on•call duties
• Weekly 1•hour IDT meeting on Mondays

**Qualifications:**

• Active PT License and BLS Certification
• 2+ years of physical therapy experience
• Home health experience preferred; outpatient/inpatient experience considered

**Key Responsibilities:**

• Conduct clinical assessments and deliver physical therapy
• Supervise PTAs with weekly office visits
• Perform Start of Care (SOC) assessments
• Engage in field shadowing and training
• Manage a caseload of 6•7 points daily

**Additional Details:**

• Initial two•day in•office orientation, then fieldwork
• Oasis training available if needed
• Mileage reimbursement for patient visits
