COVER LETTER IS REQUIRED Key Responsibilities:- Conduct psycho-social intake and clinical assessments for referred minors and their households.- Perform regular home visits to assess safety and risk, ensuring ongoing support and intervention as necessary.- Collaborate closely with families, educational institutions, and community service providers to create and implement individualized service plans.- Maintain accurate data entry in ACS and OCFS databases, including ongoing clinical case notes and Family Assessment Service Plans (FASP).- Develop strong relationships with youth and families, responding promptly to referrals and providing coverage for direct service staff as needed.
