Trillium Staffing is now hiring a In-Home Assessment Social Worker for a part time role in Bad Axe, MI!

Pay $28-31 hr. based on experience. Schedule M-F 9-6 with some flexibility.

In-Home Assessment Social Worker job duties include:

• Conduct in-home assessments to evaluate seniors' psychosocial, environmental, and functional needs, identifying risks and areas for support.
• Develop individualized care plans in collaboration with clients, families, and care teams, ensuring access to appropriate community resources and services.
• Provide ongoing support and advocacy, including referrals, education, and coordination with healthcare providers and agencies to promote aging in place safely.

In-Home Assessment Social Worker Qualifications:

• Bachelor's or Master's degree in Social Work (BSW/MSW) and active licensure (LBSW, LMSW, or equivalent) in the state of [Insert State].
• 12 years of experience working with older adults, preferably in home-based, community, or healthcare settings.
• Strong clinical assessment, communication, and case management skills, with the ability to work independently and travel to client homes.
