\tModels a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers\tIdentify medically related social and emotional needs of residents, through individual and group social services programming\tPromote residents potential during their stay\tCompletes psycho-social assessment, identifies social and emotional needs of residents and then develops a care plan for each resident which identifies medically related social and emotional problems, identifies needs on discharge planning when applicable and needs for primary assigned areas\tParticipates in interdisciplinary care plan conferences and contributes to the development of care plans and discharge planning if applicable for residents for primary assigned areas\tDesigns and develops social service programs that meet medically related social and emotional needs of residents and complies with regulatory guidelines\tAssesses needs for ancillary services (dental, podiatry, optometry, psychological, etc.) and assists residents in attaining needed services for primary assigned areas\tAssist with interpreting and promoting Resident Rights and Resident Bill of Rights\tMaintain accurate and timely documentation (initial assessment, program notes, advanced directives, MDS, mini-mental State exams, depression scale, discharge documentation, etc.) for primary assigned areas\tMaintains Grievance Log, Satisfaction Surveys and Missing Items Log, for assigned units, and follow-up with residents for resolution, summarize monthly for quality assurance and report any concerns to Administrator of primary assigned areas\tProvides individualized support for residents and families in times of adjustment, crisis or needs\tOrients residents and families to the facility environment and provide support in adjusting\tCoordinates discharge planning with teammates, residents and families for primary assigned areas\tMaintains professional qualifications by obtaining required CEUs annually\tPursues professional development in social services disciplines through workshops, readings, and membership in appropriate organizations\tProvides information on hospice as appropriate and assists resident/family with initiation of services for primary assigned areas\tEnsure HIPAA P&Ps are being followed and report to HIPAA officer any concerns\tAttend all committee meetings\tThoroughly investigate any complaints and concerns and report them to the Administrator\tProvide social service support outside primary area as needed or assigned by Administrator\tOther duties as assigned by AdministratorMINIMUM QUALIFICATIONS:\tLicensed by the Texas State Board of Social Worker Examiners\tBachelors degree in social work (BSW), or secondary education in the social services and certification as a social work "designee may be substituted as appropriate\tTwo years of social work supervised experience in a LTC setting preferred
\tLTC, Medicare, Medicaid, Discharge Planning experience required
\tEffective interpersonal and communications skills
\tFunctional literacy in English
\tAbility to perform Essential Duties\tGenuine caring for and interest in the care of mature adults, people with handicapped and hospice patients
