About the Role: The RN Care Manager plays a crucial role in coordinating patient care and ensuring that individuals receive the necessary medical services to achieve optimal health outcomes. This position involves assessing member's needs, developing personalized care plans, and collaborating with healthcare teams to implement these plans effectively. The RN Care Manager will also monitor a member's progress, adjust care strategies as needed, and provide education and support to members and their families. By fostering strong relationships with members and healthcare providers, the RN Care Manager helps to navigate complex healthcare systems and improve overall member satisfaction. Ultimately, this role is vital in promoting health, preventing complications, and enhancing the quality of life for members in the community. Minimum Qualifications: Current Registered Nurse (RN) license in the state of Florida. Bachelor's degree in Nursing (BSN) or higher. At least 2 years of clinical nursing experience, preferably in a care management or similar role. Relevant experience may substitute for the educational requirement on a year-for-year basis. Preferred Qualifications: Certification in Care Management (CCM) or similar credential. Experience with electronic health records (EHR) and care management software. Knowledge of community resources and healthcare services available in Osceola, FL. Responsibilities: Conduct comprehensive assessments of patients' health status and needs. Develop, implement, and evaluate individualized care plans in collaboration with patients and healthcare teams. Coordinate and facilitate communication among various healthcare providers to ensure continuity of care. Monitor patient progress and outcomes, making necessary adjustments to care plans as required. Provide education and resources to patients and their families to empower them in managing their health. Skills: The required skills for this position include strong clinical assessment abilities, which are essential for evaluating patient needs and developing effective care plans. Excellent communication skills are vital for collaborating with healthcare teams and educating patients and families about their care. Organizational skills are necessary to manage multiple patient cases and ensure timely follow-up on care plans. Problem-solving skills are also important, as the RN Care Manager must adapt care strategies based on patient progress and changing health conditions. Preferred skills, such as familiarity with EHR systems, enhance the efficiency of documentation and information sharing, ultimately improving patient care coordination.
