What We Offer: Competitive Pay Flexible Scheduling Career Development Opportunities Position Details: This part-time role requires conducting home visits in the Highlands & Hardee County area, offering one-on-one patient care in the comfort of their home. You will execute physician orders and keep them updated on any significant changes in your patient's condition. As a Physical Therapist, you will assess, plan, implement, and evaluate personalized physical therapy care for each patient. Responsibilities Include: Complying with all departmental policies and standards of care. Performing comprehensive initial evaluations on all assigned patients requiring physical therapy. Conducting home safety assessments and communicating specific needs to the physician and team members. Developing a tailored plan of care with goals based on the patient’s functionality and potential for improvement, in conjunction with the patient, family, or caregiver. Implementing the physical therapy plan and continuously assessing the patient's response to the program. Reporting observations to the attending physician and making necessary adjustments to the care plan. Your Qualifications: Minimum Requirements: Current Physical Therapy licensure in Florida American Heart Association Basic Life Support (BLS) certification Valid Driver's License BS in Physical Therapy from an accredited American Physical Therapy Association program At least 1 year of clinical Physical Therapy experience in an acute or rehab setting Preferred Qualifications: MS or DPT in Physical Therapy from an accredited American Physical Therapy Association program 2 years of experience in Home Health Physical Therapy We are an equal opportunity employer that complies with all anti-discrimination laws.
