Nurse Practitioner / Physician Assistant – Per Diem

We are seeking an experienced and independent Nurse Practitioner or Physician Assistant to provide advanced wound care services in patients’ private residences throughout the Inland Empire, primarily within San Bernardino County and surrounding communities.
As a specialty wound care practice, we focus on delivering evidence-based treatment that improves healing outcomes and reduces avoidable hospitalizations.
This mobile, per diem role is ideal for providers who value autonomy, flexible scheduling, and meaningful one-on-one patient care outside of traditional hospital or facility environments.
Minimum availability of 4-5 days per week required.
Position Overview
· Per Diem (as-needed basis)
· Home-based patient visits
· Independent, field-based clinical practice
· Travel within assigned Inland Empire territory
· Flexible scheduling based on availability
Clinical Responsibilities
· Conduct comprehensive in-home wound assessments
· Develop and implement individualized, evidence-based treatment plans
· Perform advanced wound care procedures, including debridement and application of appropriate dressings
· Monitor healing progression and adjust care plans appropriately
· Evaluate and manage basic dermatologic conditions within scope of practice
· Educate patients and caregivers on wound management, infection prevention, and offloading strategies
· Complete accurate and timely EMR documentation
· Implement established company protocols for clinical care and documentation
Qualifications Required:
· Active, unrestricted California NP or PA license
· Current DEA registration
· Current BLS certification
· Minimum 1 year of clinical experience
· Reliable transportation and ability to travel locally
Preferred:
· Wound care experience
· Experience with lower extremity or vascular assessment
· Proficiency with EMR documentation systems
· Flexible with scheduling and travel within assigned service areas
· Ability to reposition or turn patients as needed without guaranteed assistance
· Bilingual (English/Spanish)
Service Area – Inland Empire
This position primarily serves central Inland Empire communities, including:
Redlands, Loma Linda, Moreno Valley, Riverside, Corona, San Bernardino, Colton, and Grand Terrace.
Expanded coverage opportunities may be available for providers open to additional territory (e.g., Rancho Cucamonga, Ontario, Fontana, Upland, and select High Desert areas).
Territory assignments are structured to promote efficient routing and minimize unnecessary travel whenever possible.
Compensation
Part Time and Full Time Per Diem Structure starting at $75.00 per hour
