Position Overview The PTA will work collaboratively with our in-house therapy team to deliver individualized care plans that promote independence, safety, and quality of life for our patients in the home setting. This role offers a manageable caseload, local travel, and the opportunity to build meaningful relationships with patients and families.

Qualifications Current Ohio licensure as a Physical Therapist Assistant (required) Minimum of one year of recent PTA experience (hospital, SNF, ALF, hospice, or home health) preferred Home health experience is beneficial but not required; training available for the right candidate Strong communication skills with patients, families, and interdisciplinary care teams Ability to work independently and manage time effectively
