JOB SUMMARY Provides home health services consistent with the mission, vision and goals of the Agency. Physical Therapy Assistant is a member of the home health team who assists in the provision of physical therapy and the coordination of patient care under the supervision of a Physical Therapist to provide treatment of disabilities, diseases, or other conditions of health by the use of physical, chemical, and other properties of air, cold, heat, electricity, exercise, massage, ultrasound, water or equipment in accordance with a written plan of treatment prescribed by a physician.
DUTIES AND RESPONSIBILITIES Perform services planned, delegated and supervised by the Physical Therapist that are ordered by the physician as indicated in the plan of care Contributes to the ongoing interdisciplinary assessment of the patient in collaboration with the supervising Physical Therapist. Assess ability or disability, relieve pain, prevent disability and/or further deformity, develop or improve body skills, and restore function and maintain maximum performance within the patient's capabilities. Assess and perform procedures such as manual and electrical muscle examinations, functional evaluations, gait analysis, sensory testing, measurements of range of motion, and assessment of development and utilization of human movement. Observe, record and report to the supervising Physical Therapist, all physicians involved in the plan of care, and other health care professionals (as appropriate) related to the current plan of care, including the patient's reaction to treatment and any changes in the patient's condition. Instruct patient in use of wheelchairs, braces, canes, crutches and prosthetic and orthotic devices. Participate in teaching / education and counseling of the patient, family and caregivers. Participate in Agency-sponsored in-service education, utilization review and team conferences. Assist in updating the plan of care, in collaboration with the supervising Physical Therapist, the patient, representative (if any), and caregivers. Prepare and submit all clinical case documentation, including clinical and progress notes, and required payroll information according to Agency policy. Document each treatment in the patient's record and notify the supervising physical therapist of any change in the patient's condition not consistent with planned progress or treatment goals. Ensure that the supervising Physical Therapist reviews and cosigns all documentation by the Physical Therapy Assistant. Provide services within the Agency's catchment area. Acknowledge patient rights including privacy and confidentiality. Consistently display a professional appearance. Interact with internal and external customers using positive and professional approach. Assume responsibility and display reliability for the completion of all tasks, duties, communications and actions. Maintain professional growth and development. Participate in the Agency's QAPI Program for performance improvement activities. Abide by Agency policies and procedures. Accept other duties as assigned.
PHYSICAL DEMANDS AND WORKING ENVIRONMENT Must have visual and hearing acuity. Communicates verbally in a clear fashion.. Must be able to read, write and speak English fluently. May be exposed to minor hazards/communicable diseases. Must provide proof of auto insurance upon hire and each renewal and maintain a safe driving record.
EDUCATION, EXPERIENCE, AND TRAINING Education and qualifications commensurate with ยง 484.115(i) (Condition of participation: Personnel qualifications / Physical Therapy Assistant). Current, active license as a Physical Therapist Assistant by the Physical Therapy Examining Committee of the California Board of Physical Therapy. Have graduated from a Physical Therapy Assistant program approved by the Board and has successfully passed the examination required by the Board governing physical therapy.
