Overview We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will possess a strong background in behavioral health and social work, with a focus on providing support and resources to individuals in need. As a Case Manager, you will play a crucial role in crisis intervention, workforce development, and child welfare, ensuring that clients receive the appropriate services to improve their quality of life. Responsibilities Conduct comprehensive assessments of clients to identify needs and develop individualized service plans. Provide crisis intervention and management to clients facing immediate challenges. Collaborate with healthcare providers, social services, and community organizations to coordinate care. Utilize motivational interviewing techniques to empower clients in their recovery journey. Monitor client progress and adjust service plans as necessary to ensure effective outcomes. Offer mentoring and support to clients navigating addiction counseling or behavioral health issues. Maintain accurate documentation of client interactions, progress notes, and service outcomes. Stay informed about Medicare regulations and other relevant policies impacting client services. Experience A degree in Social Work, Psychology, or a related field is preferred. Previous experience in crisis management or behavioral health settings is highly desirable. Familiarity with workforce development initiatives and child welfare practices is a plus. Strong interpersonal skills with the ability to build rapport with diverse populations. Knowledge of motivational interviewing techniques and best practices in case management. Experience in mentoring or providing support to individuals facing addiction challenges is beneficial.
