Position: Home Health Occupational Therapist (OT) We're looking for a dedicated full-time OT to provide personalized, one-on-one care to patients in their homes. What You'll Do: Empower Daily Living: Conduct thorough patient evaluations and develop individualized treatment plans based on physician orders. Restore Independence: Assist patients in improving daily living activities and functional mobility through targeted interventions. Optimize Home Environments: Provide hands-on therapy, including adaptive equipment training and home modification recommendations, to enhance safety and accessibility. Educate and Support: Provide comprehensive education to patients and families on treatment procedures and home exercise programs. Drive Progress: Continuously reassess and modify treatment plans to maximize patient outcomes. Collaborate for Success: Work closely with physicians and a multidisciplinary team to ensure seamless, coordinated care. Maintain Excellence: Ensure accurate and timely documentation using Epic EMR. What You'll Bring: Bachelor’s Degree in Occupational Therapy (Required). Active Illinois Occupational Therapist License in good standing (Required). Valid Driver’s License (Required). Schedule & Flexibility: Monday – Friday, (8:00 AM – 4:30 PM) with occasional weekend and holiday rotation. 4-10s available! Centralized territories to minimize travel. Compensation & Benefits: Competitive Salary: $65,000 - $110,000 per year (commensurate with experience). $2,500 Sign-On Bonus! (Paid in the 2nd pay period). Generous Mileage Reimbursement (IRS rate).
