CenterWell Home Health is seeking a compassionate and motivated Occupational Therapist, Home Health to deliver high-quality, patient-centered care in the comfort of our patients’ homes. In this role, you will provide skilled occupational therapy services to adult and geriatric patients, focusing on improving safety, independence, and quality of life in the home environment. You will conduct comprehensive assessments, develop personalized treatment plans, and implement evidence-based interventions to help patients perform activities of daily living, manage functional limitations, and safely navigate their home surroundings.

As a key clinical partner, you will collaborate closely with physicians, nurses, physical therapists, speech therapists, social workers, and caregivers to ensure coordinated, holistic care. You will educate patients and families on adaptive techniques, home exercise programs, energy conservation, fall prevention, and the proper use of assistive devices and durable medical equipment. You will regularly reassess patient progress, adjust care plans based on clinical findings, and document all services accurately and timely in accordance with CenterWell Home Health policies, payer requirements, and regulatory standards.
