Overview
HOPE Community Services, Inc. is a leading outpatient treatment provider in south Oklahoma City.
We provide compassionate trauma-informed care to individuals whose lives have been impacted by mental health and substance use disorders. We do this by providing compassionate and evidence-based treatment, aimed at improving the quality of life for those we serve.
We are currently seeking an LPN Nurse Care Manager to join our team. This position focuses on the assessment, management, and coordination of client care, including medication management and health screenings. It is ideal for an LPN looking to work in a collaborative, recovery-focused environment that emphasizes client-centered care.
About the Position
As an LPN Nurse Care Manager at HOPE Community Services, you will be responsible for providing comprehensive primary care and nursing services to clients. Your role will involve conducting assessments, creating treatment plans, and managing chronic conditions like diabetes, hypertension, and other major health concerns. You will work closely with the treatment team, coordinating services with external providers, and ensuring clients have access to the support they need for successful recovery.
Key responsibilities include conducting health screenings, educating clients on medication use and chronic disease prevention, managing follow-up care, and ensuring clients have access to benefits such as Social Security, Medicaid, and insurance coverage. You will also maintain accurate clinical records and engage in regular case collaboration to ensure high-quality, integrated care.
About the Role
The LPN Nurse Care Manager plays an essential role in HOPE Community Services integrated care model. This position is part of a collaborative team that works together to offer comprehensive services, including primary care, mental health, and substance use recovery. The role also requires ongoing communication with other healthcare professionals to ensure continuity of care and effective treatment planning.
Requirements
Current LPN license in Oklahoma
Experience in community-based mental health care
Knowledge of primary care services, chronic disease management, and crisis intervention
Ability to work collaboratively in a trauma-informed, recovery-focused, and culturally competent environment
Skills
Strong clinical and organizational skills
Ability to conduct health assessments and manage care for individuals with chronic conditions
Excellent communication and documentation skills
Compassionate, client-centered approach to care
Ability to work in a team-based setting and collaborate with multiple healthcare providers
Additional Requirements
May require Therapeutic Options training, First Aid, CPR, and Wellness Coach Certification
