Mount Carmel St. Ann’s is seeking detail-oriented and patient-focused Medication Historians to join our pharmacy team in a hybrid role that combines both remote and onsite responsibilities. Certified Pharmacy Technicians in this position will practice at the top of their certification by playing a critical role in patient safety and care coordination. Medication Historians are responsible for compiling accurate and comprehensive medication histories for patients upon admission, working directly with patients, caregivers, external pharmacies, and electronic databases. This role includes remote support for medication history documentation as well as onsite responsibilities at Mount Carmel St. Ann’s, where team members will engage in direct patient interviews and collaborate with the multidisciplinary care team. Ideal candidates will demonstrate strong attention to detail, excellent communication and problem-solving skills, and thrive in a fast-paced, patient-centered environment.

Position Purpose:

The Pharmacy Medication Historian assists with compiling an accurate and complete patient medication history. Interviews patients and uses a standardized tool to record all patient medication information including prescribed medications, over-the-counter medications, dietary and herbal supplements. Works with patients/family members to maintain updated medication lists, communicates pertinent findings to appropriate care team members, and identifies and facilitates correction of medication discrepancies.
