* Board Certified in Internal Medicine or Family Medicine - REQUIRED * Board Certified in Geriatrics (Fellowship-trained) or Certificate of Added Qualifications in Geriatric Medicine (to be completed prior to start) - HIGHLY PREFERRED * IN license (or willing to obtain) - REQUIRED
* Clean Malpractice - HIGHLY PREFERRED
* CDS Certification (by time of start) - REQUIRED * DEA Verbal Confirmation (by time of start) - REQUIRED * BLS (by time of start) - REQUIRED * Minimum of 1 year experience working with the frail or elderly population (add to "Experience" field) - REQUIRED * Previous supervisory experience (add to "Experience" field) - REQUIRED * Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED
