We are seeking Licensed Practical Nurses (LPNs) experienced in behavioral health and enhanced care who can support complex needs in a 2:1 care setting through in-home and community supports. Ideal candidates will hold or be pursuing NADD or NADSP certification, possess strong behavioral insight, and provide skilled services such as G-Tube feeding, medication administration, and emergency response in community-based home settings.

Key Responsibilities Clinical & Behavioral Health Care Deliver skilled nursing services in a 2:1 home and community support setting per individualized service plans (ISP) Provide G-Tube care, administer oral and enteral medications, and support hygiene and safety needs Implement behavioral support strategies and manage crises using trauma-informed approaches Monitor and document vital signs, behavioral patterns, and health changes through Electronic Visit Verification (EVV) Maintain a safe and structured environment while encouraging independence and person-centered routines
Care Coordination & Documentation Collaborate with physicians, behavior specialists, support coordinators, and families to ensure continuity of care Support DSPs with hands-on training in health protocols, medication support, and behavior management Ensure full compliance with HIPAA and Pennsylvania Chapter 6100 regulations, particularly for in-home and community supports Attend appointments and follow up with updates and reporting through the appropriate documentation systems
Training, Advocacy & Development Maintain active NADD or NADSP Certification, or be open to earning credentials through supported programs Participate in regular training in medication administration, G-Tube management, positive behavior support, and crisis prevention Empower families with education on behavioral triggers, medication schedules, and safety best practices
Minimum Requirements Active LPN License in Pennsylvania or Multi-State License (MSL) Required 25 years of experience in IDD care, behavioral health, or enhanced support services NADD or NADSP Certification (or willingness to obtain) CPR/BLS Certification Required G-Tube and medication administration experience Required Clean background check, drug screen, and 3 professional references Valid drivers license, reliable vehicle, and current auto insurance
