YOU ARE/POSSESS: - Excellent written and verbal communication skills- Licensed Registered Nurse - Prior MDS and Medicare skilled nursing preferred - Experience supervising others preferred - Must demonstrate the ability to work responsibly as a team member as well as an individual DUTIES: - Other duties as assigned- Coordinate the development and completion of the resident assessment (MDS) - Work closely with the Director of Nursing Services to assess and measure health outcomes for residents - Coordinate with physicians, healthcare providers, family members, nursing staff, and facility inter-disciplinary teams to provide holistic care to residents - 4-star quality measure for short-term stay residents: proven track record of improvement in resident function, and above state and national average for rate of successful return home after therapy and care. - Staffing levels: in addition to required nursing staff, we employ an additional CNA on each shift above the minimum staffing ratio. - Part of the larger health district: includes in-home care, assisted living, and a memory community, to allow seamless transition for our residents as their needs change.
