Position Overview: IHCC & Non-Medical Home Assessments As an IHCC LPN, you will combine clinical knowledge with holistic home assessments, ensuring clients receive the care and resources they need. You will perform in-home visits, coordinate services, and assess living conditions to create safe and supportive care environments.

Key Responsibilities: Conduct non-medical home assessments to evaluate living environments, safety needs, and support systems. Deliver in-home nursing care, including health checks, medication support, and chronic disease monitoring. Collaborate with physicians, therapists, and case managers to update and maintain care plans. Document all visits and care plans using Electronic Visit Verification (EVV) systems in compliance with Indiana Medicaid Waiver guidelines. Educate clients and families on disease prevention, wellness strategies, and medication adherence. Complete a minimum of two in-person visits per client per month (1 hour minimum each), with supplemental outreach calls or telehealth follow-ups as needed.

Qualifications Active LPN license in the State of Indiana (required) CPR certification (or willingness to obtain) 12 years of experience in home health, community nursing, or case management preferred Knowledge of Indiana Medicaid Waiver Programs (A&D, TBI, IHCC, SFC) a plus Ability to work independently, conduct home safety assessments, and complete thorough documentation Compassion, attention to detail, and excellent communication skills
