SUMMARY: Under the supervision of the Registered Nurse, the Licensed Practical Nurse (LPN) will assist with medication administration, taking and recording vital signs, and maintain accurate medical files. Assist participants with personal care and rehabilitative services. Will also assist with program, recreational and social activities, including lunch, as needed to meet the needs of the participants. The LPN shares responsibility with all staff for assisting clients as needed, reporting to the Registered Nurse. RESPONSIBILITIES: Direct Service Administer treatments and medication, in absence of the RN. Provide hands-on assistance with toileting Assist participant in self-care activities Assist with serving lunch and snacks, monitoring individual dietary needs Working in collaboration with the Social Worker, partake in new participant intake and facilitate nursing assessments. Collaboration with other staff Communicate care needs of participants, suggest improvements and discuss progress with the Center’s Care Team. Report changes in client condition. Assist with maintenance of clinical records; assuring required documentation is in patient charts to assure regulatory compliance Escort on bus route and field trips as needed. REQUIREMENTS LPN license in the District of Columbia Experience working as with geriatric population preferred Ability to communicate with and establish effective relationships with staff, participants, caregivers and volunteers Flexibility in schedule and open to short notice requests to cover Ability to lift at least 20 pounds Other requirements: Initial and annual chest X-ray/PPD; CPR and First Aid certification; Maintain Practical Nursing License
