Position Purpose: The Licensed Clinical Social Worker (LCSW) plays a vital role in supporting the emotional and behavioral health of residents at Crocker Residential, a licensed Residential Care Facility for the Elderly (RCFE) serving individuals with a history of homelessness. This full-time, in-house position focuses on mental health care, substance use recovery support, crisis intervention, and care coordination. The LCSW collaborates closely with administrators, resident aides, outside providers, and community agencies to ensure residents receive comprehensive, trauma-informed support that honors their dignity and enhances quality of life. In addition, the LCSW is responsible for helping residents navigate Medi-Cal benefits and behavioral health resources under the Cal AIM initiative. Program Population: The Crocker Residential Care is a licensed Residential Care Facility for the Elderly (RCFE) serving older adults, including individuals with histories of homelessness, substance use disorders, behavioral health challenges, and/or chronic medical conditions. Residents may require support accessing community resources, engaging in recovery, or managing psychiatric symptoms in a supportive housing environment. Principal Responsibilities: Conduct comprehensive psychosocial and behavioral health assessments upon admission and as clinically indicated. Provide crisis intervention and crisis stabilization which may include initiated a 5150 hold and working with County Mental Health regarding timely disposition. Develop and implement individualized care plans addressing mental health, substance use, trauma history, and recovery goals. Provide direct clinical services including supportive counseling, crisis intervention, harm-reduction support, and relapse prevention strategies. Monitor resident behavioral health status and intervene in decompensation or high-risk situations. Collaborate with program staff to ensure trauma-informed, person-centered practices are integrated into daily care. Coordinate with County Mental Health, local outpatient services, and Medi-Cal Managed Care Plans for behavioral health referrals and continuity of care. Assist residents in understanding and accessing benefits through Medi-Cal and CalAIM programs (e.g., Enhanced Care Management). Maintain confidential, timely, and accurate documentation in compliance with RCFE and County standards. Participate in resident care team meetings, quality assurance reviews, and staff training on behavioral health best practices. Requirements: License: Must hold a current, active Licensed Clinical Social Worker (LCSW) license issued by the California Board of Behavioral Sciences. Must be Lanterman-Petris-Short (LPS) designated to initiate 5150 holds. Education: Master’s degree in Social Work (MSW) from an accredited institution. Clinical Knowledge: Demonstrated experience or training in trauma-informed care, co-occurring disorders (mental health + substance use), harm reduction, and housing-first models. Regulatory Knowledge: Familiarity with Medi-Cal, CalAIM Enhanced Care Management, and applicable RCFE and HIPAA regulations. Skills: Excellent interpersonal, crisis de-escalation, and written communication skills. Ability to work with residents from diverse backgrounds in a respectful, empowering manner. Additional Requirements: Must clear a DOJ/FBI criminal background check, physical exam, and TB screening prior to hire. Must maintain annual training compliance per CCLD standards. CPR and First Aid certification required or obtained within 30 days of hire.
