The Intensive Community Care Nurse Manager is a Registered Nurse who collaborates with our most complex patients, their primary care physicians, and within a dedicated care team. Your role is essential in providing focused case management and field nursing interventions aimed at preventing unnecessary hospital admissions and maximizing the healthy time our patients spend at home. In this impactful position, you will lead the Complex Care Team, playing a vital role in assessing, evaluating, and coordinating care for high-risk patients. This includes ensuring safe transitions from facilities back to our care teams, stabilizing ambulatory patients, and reaching out to those who are not engaged in their care. You will develop comprehensive care plans, supervise team members, and help drive actions to maintain patient health at home. Key Responsibilities: Conduct in-home, facility, and telephonic visits to high-risk patients to prevent unnecessary hospital admissions. Perform detailed assessments and develop individualized care plans, identifying patient goals and barriers. Conduct supervisory visits to oversee care provided by LPNs and ensure proper patient discharge from case management. Implement clinical assessments, medication monitoring, and health education to promote self-care. Coordinate the overall plan of care, ensuring it reflects patients' needs and available community services. Facilitate patient access to healthcare and community resources, offering support and navigation guidance. Maintain ongoing communication with families and community providers to enhance patient well-being. Monitor the quality and appropriateness of health service visits and interface with multidisciplinary teams to ensure comprehensive care. Foster strong relationships with patients that promote self-management and independence. Qualifications: Associate degree in Nursing is required; a Bachelor's Degree preferred. Valid, active Registered Nurse (RN) license; Compact License preferred. Minimum of 2 years clinical experience, with at least 1 year in community case management preferred. Preferred certification in Case Management through recognized organizations. Excellent interpersonal, communication, and critical thinking skills. Ability to work autonomously and make independent clinical judgments.
