Description
Position Title: Patriot Care Oversight Registered Nurse Job Type: As needed only Pay: $55 per visit Schedule: Flexible. Day and afternoon hours needed. Travel within The Southeast Michigan region as required Patriot Care, a member of the Kennedy Care family of companies, is based in Ann Arbor, Michigan, providing home care services to Veterans across the state of Michigan, Wisconsin and Ohio.
As we continue to grow, we're building a team of nurses to oversee that the services we provide meet the needs of every Veteran and are delivered at the standard they've come to expect.
The Oversight Nurse is responsible for participating in regularly scheduled oversight visits, and ensuring that every Care Plan is accurate, built to provide our Life Care Professionals (caregivers) with clear instructions on how to care for their client.
This unique role provides you (an experienced Registered Nurse) the opportunity to contribute to the lives of Veterans while maintaining total control of your schedule.
Job Responsibilities Build and maintain meaningful relationships with Veterans and their families. Complete in-home care assessments for Veterans on a routine basis to review the care needs, ensure the accuracy of the Veterans care plan, ensure that the care being delivered by Kennedy Care Life Care Professionals meets the expectation of the Veteran, and communicate with the nurse supervisor any necessary revisions or areas of concern. Review Veteran care plans to ensure accuracy and work with the Director of Nursing to update. Complete quality assurance assessments as required. Communicate any observed significant changes in condition, incidents, and report any other concerns to the supervisor as necessary. Document any needed updates to and visit outcomes in a timely manner as necessary. Work with clients and families to schedule ongoing visits in accordance with VA guidelines. May work alongside the Director of Nursing in regards to completion of new client intakes. Use of electronic software Follow all policies and procedures regarding service delivery and documentation. Requirements License current and in good standing in the state of Michigan, Wisconsin or Ohio as a Registered Nurse (RN) Access to equipment necessary to complete required documentation (smartphone or computer) Must possess and maintain current CPR Certification. Must have reliable transportation and updated driver records (valid driver's license, registration and insurance). Willing to travel 30 miles from home to see Veterans in their homes. Verbal and written communication skills Prior experience working with Electronic Medical Records (EMR) Previous home care experience preferred
