POSITION: HOUSING CASE MANAGER

REPORTS TO: PROGRAM MANAGER CLASSIFICATION/FLSA: FULL-TIME/NON-EXEMPT

Position Summary

The Housing Case Manager supports families experiencing homelessness by assisting them to rapidly access permanent housing and supportive services to achieve long-term housing stability. The position provides case management services to address multiple complex needs and reduce barriers, including referrals to community resources and linkages to other supportive services such as child care, employment services, health and wellness services, life skills workshops, and aftercare services to support stability. This position will work in our Interim Housing facility located in Culver City.


ESSENTIAL RESPONSIBILITIES:


  • Complete intake and assessment on all participants identified to determine most appropriate level of housing

  • Identify and assist participants in securing housing, basic needs, medical and mental health services, substance abuse treatment, and public benefits where eligible; develop a Housing Stability or Individual Service Plan in conjunction with the

  • Provide strengths-based case management and service coordination designed to assist participants in obtaining and maintaining stable

  • Meet with participants on a weekly basis (more often when needed), to monitor progress and /or barriers toward achieving their Housing Stability or Individual Service

  • Utilize evidence-based practices in service delivery such as Trauma-Informed Care, Intensive Case Management, Motivational Interviewing, Harm Reduction, and Housing First

  • Provide crisis intervention services as neede

  • Maintain accurate and timely records of activities, case management notes, and services provided to each client using HMIS and any other applicable data management

  • Develop and maintain relationships with private, public, and nonprofit landlords, including municipal housing departments, in order to access housing

  • Perform site visits of prospective units to ensure they meet guidelines for habitability

  • Ensure subsidized units are within the Fair Market Rate (FMR) guidelines; negotiate rent when

  • Provide advocacy as needed to help address issues and barriers between landlord and participant whenever

  • Develop and maintain relationships with participants, staff and local service



  • Attend staff and other community meetings as necessary; and serve as a representative of UBH at community

  • Prepare and present assigned cases at weekly/monthly Participant Progress Review supervision

  • Provide regular, written reports to the Program Manager and Management as

QUALIFICATIONS:


  • Bachelors Degree, from an accredited College or University in Human Services (e.g., psychology, social work, sociology or related field) required.

  • Minimum of two-years experience providing services to persons experiencing homelessness, preferably families with minor

  • Exhibit high level of

  • Demonstrated experience in working with people experiencing behavioral health issues, such as mental health, substance use, sex trafficking, or special

  • Experience working with a diverse population, with sensitivity to differences in culture, religion, sexual orientation, race, age, and physical and mental

  • Understanding of best practice approaches including: Housing First, Motivational Interviewing, Harm Reduction, and Trauma-Informed

  • Knowledge of community

  • Strong written and verbal communication

  • Ability to maintain personal and professional boundaries

  • Knowledge of Trauma-Informed Care and Practices a

  • Ability to work collaboratively, but

  • Highly motivated self- starter with the ability to work in a high-pressure

  • Creative problem-solving

  • Ability to respond responsibly in emergency and crisis

  • Exceptional organizational

  • Bilingual (English/Spanish)

  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook).
