Today, AIDS Outreach Center (AOC) began as a grassroots organization responding to the HIV epidemic. Since that time, AOC has been a leader in North Texas in the fight against HIV. Annually, 42 staff members provide services and education to over 2,000 people living with HIV and prevention services to over 3,500 people in our community.

Responsibilities
Position Summary:
Under the Direction of Housing Services, the Housing Case Manager provides client- centered, comprehensive, community-based housing case management services to individuals living with HIV/AIDS and their families participating in HOPWA funded programs. This role delivers client centered case management aimed at achieving housing stability, reducing barriers to permanent housing, and improving health outcomes. l
Primary Responsibilities:
Conduct comprehensive needs assessments to evaluate client functioning across key domains: basic needs, housing stability, physical and mental health, social support, substance use, financial/benefits access, legal issues, cultural or language needs, employment, and minor dependents.
Develop individualized comprehensive housing plans in collaboration with the client,
incorporating input from family members, significant others, and other providers as appropriate. Plans should promote increased self-sufficiency and functioning across life domains.
Assist clients with completing financial assistance applications; prepare and submit check requests to internal financial staff; and coordinate the timely processing and payment of housing and other supportive financial assistance.
Follow up on referrals to internal and external service providers to ensure client access
and participation, and coordinate services to facilitate an integrated and seamless delivery of care.
Monitor client progress, update housing plans as needed, and maintain regular client contact in accordance with programmatic standards and client need.
Review client files regularly to ensure compliance with organizational policies and established standards of care.
Provide informal counseling and support in life skill areas such as activities of daily living, resource navigation, and coping strategies to promote long-term
Accurately document all services provided and maintain complete and confidential client records in accordance with employers' policies and procedures.
Conduct regular home visits and community-based case management focused on housing retention, engagement in medical care (including HIV primary care), access to
mental health and substance use services, benefits enrollment, and other stabilizing supports.
Work collaboratively with landlords, property managers, and housing partners to resolve
housing crises, support client tenancy, and prevent evictions.
Ensure all documentation and data entry meet the reporting and compliance standards
required by Ryan White, HOPWA, and other applicable funding sources.
Perform other related duties as assigned by the Director of Housing
