Position Summary:
The Hospice LPN - On-Call (PRN) provides essential after-hours care and support to hospice patients and families in their time of need. This role exists to supplement primary on-call coverage by providing flexible, compassionate, and timely support during evenings, weekends, and holidays. This position is designed to share the on-call responsibilities with the primary on-call nurse, ensuring staff well-being while maintaining the highest standard of care for patients and families.

Key Responsibilities:

Respond to patient needs and symptom management concerns during assigned on-call hours
Perform home visits as required to assess patient condition, administer medications, and provide interventions within scope of practice
Communicate changes in patient condition to the RN case manager and/or interdisciplinary team as appropriate
Support patients and families with education, reassurance, and emotional support during crisis moments
Collaborate with the hospice team to coordinate care and ensure continuity
Document all visits and communication accurately and promptly in the electronic medical record
Participate in a rotating PRN schedule to provide coverage for planned time off or high-volume call periods

Qualifications:

Licensure: Active LPN license in the State of Oklahoma, in good standing
Experience: Minimum 1 year clinical nursing experience; hospice or home health experience preferred
Strong clinical judgment and ability to respond calmly and effectively in crisis situations
Excellent communication and time management skills
Comfortable working independently in patient homes or facilities during after-hours
Reliable transportation, valid driver's license, and proof of auto insurance

Working Conditions:

PRN/on-call schedule with flexible coverage based on agency needs and staff availability
Field-based position; must be able to travel to patient homes/facilities as needed
Able to work nights, weekends, and holidays as requested
