Compensation:$48 - $53 hourly
Responsibilities:- Assess patients’ conditions with an emphasis on pain and symptom management.
- Develop individualized care plans in partnership with the interdisciplinary team and the patient’s family.
- Update care plans regularly to reflect changing needs and ensure the highest quality of life.
- Monitor patient responses to treatment and adjust interventions as needed.
- Coordinate closely with physicians, nurses, and other team members to provide comfort-focused care.
- Offer compassionate, emotional, and spiritual support to patients and families throughout the end-of-life journey.
- Deliver care that honors each patient’s cultural and spiritual values.
- Participate in team meetings to review care plans and address any concerns.
- Collaborate with social workers, chaplains, aides, and other support staff to ensure holistic, comprehensive care.
- Communicate with outside providers to maintain continuity of care and support for families.
- Educate patients and families about the hospice process and available services.
- Document care accurately and maintain current, detailed patient records.
- Comply with all New Jersey hospice regulations and Spero Hospice policies.
- Active Registered Nurse (RN) license in the state of New Jersey.
- 2–3 years of clinical nursing experience required; hospice, palliative care, or related experience strongly preferred.
- Solid understanding of hospice philosophy and end-of-life care principles.
- Exceptional communication and interpersonal skills to support patients, families, and the care team.
- Demonstrates empathy, compassion, and emotional sensitivity when working with individuals and families facing serious illness and loss.
- Able to work independently while contributing effectively as part of an interdisciplinary team.
- Strong time management, organizational skills, and attention to detail.
- Proficient in using electronic health records (EHR) and other healthcare software systems.
