HOME HEALTH – SPEECH LANGUAGE PATHOLOGIST We have a great opportunity for a  Part-Time  Home Health Speech Language Pathologist in the Muncie/Alexandria, Indiana area. 1:1 Patient Care Flexibility to Create Your Own Schedule Mileage Reimbursement Weekend Hours Available, if desired Service Area – Muncie, Alexandria, and surrounding areas Monitor carefully the patient status and progress toward goals, as well as compliance with physician’s orders. Responsible for educating patients and caregivers, while preparing the patient/caregiver for discharge as appropriate. Job Responsibilities Provide skilled speech language pathology in accordance with the physician’s plan of treatment. Observe, record, and report the client’s reaction to treatment and any changes in the client’s condition to the physician, DON/ Clinical Manager, according to policy. Develop, implement and revise the client’s plan of care. Promote coordination of care through interdisciplinary case conferences and communication with physician and field staff. Perform therapy interventions utilizing standard speech language pathology techniques and skills as appropriate for the condition of the patient. Education, Licensure(s) and Qualifications . Minimum of 2 year experience as a Speech Language Pathologist required. Previous Home Health experience is preferred.
