Description: About the Role At Corus Home Health in Albuquerque, we’re looking for an experienced Social Worker (MSW or LCSW) to provide compassionate, person-centered care to patients in their homes. This is a highly autonomous role where you'll have the flexibility to manage your day while making a real impact in people’s lives.

Key Responsibilities Deliver social work services to home health patients per physician orders and individualized care plans

Help patients and families navigate emotional, social, and practical challenges

Participate in interdisciplinary care planning and patient case conferences

Coordinate care with community resources and advocate for patients' needs

Educate patients, families, and team members on psychosocial issues related to illness

Document visits, care plans, and progress notes accurately and on time

Maintain confidentiality and adhere to Medicare and agency guidelines

Contribute to quality improvement and ongoing education efforts

Requirements: What We’re Looking For Master’s degree in Social Work (MSW) from an accredited program Active New Mexico license Minimum 1 year of social work experience in a healthcare setting; home health experience preferred Proven ability to work independently and exercise sound clinical judgment Strong communication and relationship-building skills Knowledge of Medicare regulations related to social work services A valid driver’s license and reliable transportation
