Job Summary and Qualifications As a Home Health Registered Nurse, you will deliver skilled nursing care to diverse patients in the comfort of their homes. You will work collaboratively with an interdisciplinary team to develop and achieve individualized patient care goals, always striving to maintain high-quality standards. Your Responsibilities Will Include: Assessing home care patients to identify their physical, psychosocial, and environmental needs through thorough documentation and evaluations. Completing OASIS assessments and visit documentation in accordance with agency policies, ensuring accurate communication and service coordination with physicians and interdisciplinary team members. Documenting significant findings and concerns while communicating pertinent changes to the Clinical Manager and other healthcare providers. Supervising and offering clinical guidance to home health aides and LPNs/LVNs, ensuring the delivery of high-quality and continuous services. Participating in on-call rotations and emergency response as per agency requirements. Qualifications Needed: Graduate from an accredited nursing program. A minimum of one year of experience in home health, public health nursing, or acute care nursing is preferred. Familiarity with Medicare home health regulations, documentation requirements, ICD-10 coding, and PPS is strongly preferred. Ability to provide reliable transportation and valid automobile insurance, along with a current driver's license. Current BCLS Certification is required before providing patient care.
