CenterWell Home Health is seeking a dedicated Home Health Physical Therapist Assistant to deliver high-quality rehabilitative care to patients in the comfort of their homes. In this role, you will collaborate closely with a licensed Physical Therapist to implement individualized treatment plans that promote mobility, independence, and overall quality of life. You will provide therapeutic exercises, gait and balance training, functional mobility activities, and patient and family education, carefully documenting each visit and patient response in accordance with regulatory and agency standards. Traveling to patients’ residences, you will assess the home environment, reinforce safety strategies, and adapt interventions to real-world daily activities, helping patients safely navigate their home and community. Working within CenterWell’s culture of compassion, integrity, and excellence, you will communicate regularly with the interdisciplinary care team, promptly report changes in patient condition, and participate in care conferences to support optimal clinical outcomes. You will be expected to manage your schedule efficiently, maintain accurate and timely electronic documentation, and adhere to all infection control and patient safety protocols.
