CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver compassionate, one-on-one rehabilitative care to patients in their homes. In this role, you will work under the direction and supervision of a licensed Physical Therapist to help patients regain mobility, improve function, and enhance their independence and quality of life. You will implement individualized treatment plans, provide therapeutic exercises and gait training, and reinforce safety and fall-prevention strategies tailored to each patient’s home environment. This position requires traveling to patient residences, conducting scheduled visits, carefully monitoring patient progress, and documenting care in accordance with regulatory and agency standards. You will collaborate closely with the supervising Physical Therapist, nursing staff, and other members of the interdisciplinary team to adjust treatment interventions, coordinate care, and support optimal clinical outcomes. As part of CenterWell’s culture of compassion, integrity, and excellence, you will educate patients and caregivers on home exercise programs, use of assistive devices, and energy-conservation techniques, while consistently upholding patient safety and dignity. You will also participate in case conferences as requested, communicate timely changes in patient condition, and contribute to continuous quality improvement efforts.
