CareStar, Inc. is currently seeking an Assessment Specialist in the Clinical department to conduct program assessments using program and CareStar identified tools and process and provide recommendations for eligibility per program and CareStar guidelines. The Assessment Specialist is a home based position that works independently to determine the eligibility of individuals who have applied for the HOME Choice program.
THE POSITION
CareStar is looking for motivated individuals who/to:
- Active license as RN, LSW, or LISW.
- 12 months experience in home and community based services within the past 5 years.
- Knowledge of Medicare, Medicaid, home health care and Ohio Home Care waiver programs desirable.
- Adheres to the CareStar Rule in performance of job responsibilities.
- Understands and complies with CareStar Policies and Procedures.
- Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
- Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
THE LOCATION
This position is open in Allen County and is responsible for covering the surrounding counties.
