Role Responsibilities Deliver comprehensive, continuous care to individuals and families, addressing their needs from childhood to elderly care. Oversee chronic conditions, preventive health, urgent medical issues, and mental well-being through a patient-first philosophy. Conduct examinations, analyze diagnostic results, and create tailored health plans. Collaborate actively with care coordinators, mental health specialists, and other healthcare professionals to provide well-rounded care. Engage in quality improvement initiatives aimed at enhancing patient results and operational excellence. Utilize electronic medical records (EMR) to ensure thorough, timely documentation of patient information. Offer flexibility in care delivery, providing both in-person and virtual consultations based on patient preferences. Education Requirements MD or DO from a recognized medical institution. Completion of a residency in Family Medicine from an accredited program. Certifications Board Certification in Family Medicine. Eligibility for (or possession of) a valid Oregon medical license. BLS and ACLS certifications preferred. Skills and Experience Exceptional diagnostic and clinical abilities with a focus on preventive care. Comfortable operating within value-based care models and enthusiastic about continuous learning. Outstanding communication and interpersonal abilities.
