A healthcare facility in Fort Edward, NY is seeking a Director of Social Work (DSW) or Director of Social Services. We’re looking for an experienced professional to lead the Social Services department. This role includes staff supervision, care planning, resident support, and coordination of discharge planning. Occasional community outreach may be part of the responsibilities.
