Embrace Your Aspiration as a Case Management Director In this role, you will: Oversee the day-to-day operations and human resource management of the Case Management department. Manage interdisciplinary plans of care and the discharge planning process, focusing on patient care outcomes and key care indicators. Advocate for patients and their families ensuring services meet their needs. Provide training and support to Case Managers and staff to enhance their skills and knowledge. Build strong relationships with insurance companies, self-insured employers, and other healthcare networks. Celebrate the successes and contributions of our dedicated team members. Qualifications CCM® or ACMTM certification is preferred. Must be qualified to independently conduct assessments within the scope of your discipline. If applicable, hold an active licensure in the hospital's state. For Nursing candidates, a bachelor’s degree in nursing (BSN) with RN licensure is required. Other healthcare professionals should possess at least a bachelor’s degree; a graduate degree is preferred. Minimum of three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning. Flexibility to work on weekdays, weekends, evenings, or holidays as needed.
