Job Summary
The RN-Clinical Coordinator plays a crucial role in coordinating multidisciplinary care across various departments, including ICU, Medical, Surgical, and Ancillary Units. This role is essential for ensuring the delivery of quality patient care.
This position involves collaboration with nursing staff, physicians, and ancillary units to assess, plan, implement, and evaluate patient care while supporting quality improvement initiatives and maintaining regulatory compliance within an acute care hospital setting.
Essential Functions
Strategically coordinate patient care across ICU, Medical, and Surgical Units to achieve optimal patient outcomes and facilitate seamless continuity of care.
Work closely with interdisciplinary teams to assess, plan, implement, and evaluate effective patient care plans.
Provide clinical guidance and support to nursing staff, serving as a crucial resource for clinical procedures, protocols, and hospital policies.
Engage in quality improvement initiatives, monitoring patient outcomes, and implementing corrective measures when necessary.
Conduct daily patient rounding to assess needs, monitor care quality, and offer support to nursing staff.
Enhance communication among medical staff, patients, families, and ancillary departments to ensure effective care coordination.
Assist with discharge planning, ensuring patients receive appropriate follow-up care instructions and necessary resources.
Design and implement educational programs that elevate clinical competencies and promote adherence to best practices within the team.
Monitor compliance with regulatory guidelines, including Joint Commission standards, EMTALA, and hospital-specific protocols.
Accurately document all patient care activities in the electronic medical record (EMR) and other clinical systems.
Perform additional duties as prescribed by the Quality Director or Chief Nursing Officer.
Knowledge/Skills/Abilities/Expectations
Exemplary clinical assessment and decision-making skills.
Outstanding communication and interpersonal abilities.
Proficient in Microsoft Office applications (Word, Excel, Outlook).
Ability to prioritize tasks and effectively manage multiple responsibilities.
Solid understanding of regulatory standards and quality improvement processes.
Adept at working collaboratively in a fast-paced, multidisciplinary team environment.
Qualifications
Education: Associate Degree in Nursing (ADN) is required.
Bachelor of Science in Nursing (BSN) or Master's in Nursing, Healthcare Management, or Administration is preferred.
Licenses/Certifications: Current RN licensure in the state of practice or compact state is required.
Basic Life Support (BLS) certification is necessary within the designated timeframe as per facility policy.
Experience: Minimum of 2 years of acute care inpatient nursing experience is required.
Experience in patient care coordination, quality improvement, and leadership is preferred.
