POSITION SUMMARY The EmPATH Registered Nurse plays a crucial role in delivering high-quality nursing and effectively managing patient care. Reporting to the EmPATH Manager, you will be responsible for providing exceptional, individualized, patient and family-centered care based on the Watson Model of Care. This role includes 12-hour accountability for a specific patient group while addressing special needs and considerations relevant to age and population. ESSENTIAL DUTIES AND RESPONSIBILITIES Provide comprehensive nursing care to mental health clients with unstable and evolving conditions. Collaborate with physicians to evaluate suicidal or homicidal tendencies, and assess for psychosis, anxiety, and depression to determine appropriate care levels. Oversee direct patient care, perform assessments, evaluate for 5150 designations, and ensure constant safety for all patients. Engage in treatment and discharge planning with physicians, families, and other vital resources. Utilize the nursing process effectively to anticipate and meet patient and family needs. Focus nursing assessments on key areas for improvement, recognizing subtle clinical changes and adapting care plans accordingly. Showcase the hospital's mission, vision, and values through interactions with patients, families, and colleagues. Develop and maintain a therapeutic nurse/patient relationship while respecting patients' rights. Conduct regular hourly rounds to ensure continuous patient engagement and monitoring. Employ the SBAR communication method during all hand-off situations to ensure clear information transfer. Document all patient care meticulously, including assessments and adherence to hospital policies. Commit to addressing the learning needs of patients and families, utilizing resources to achieve favorable outcomes. Review patient orders, promptly communicate any changes in condition with physicians, and collaborate on necessary care adjustments. Supervise and evaluate ancillary nursing personnel and registry staff to help maintain quality patient care. Adhere to a professional appearance standard, including dress code and identification protocols. Maintain a safe and regulatory-compliant environment promoting Patient Safety Goals. Contribute to the Quality and Performance Improvement Plan specific to the unit and report findings to the EmPATH Manager. Perform additional duties as assigned. POSITION REQUIREMENTS A. Education Associate's degree in nursing or higher level of nursing degree required; BSN preferred. B. Qualifications/Experience A minimum of 2 years of RN experience in acute adult outpatient or inpatient psychiatric settings, with knowledge of psychiatric and medical-surgical conditions and treatments. Current California Nursing license. Completion of AVADE training within 90 days of hire. Current Basic Life Support (BLS) certification. C. Special Skills/Knowledge Bilingual skills (Spanish) preferred. Basic computer and electronic medical records skills preferred.
