BayMark Health Services is seeking a dedicated Chemical Dependency Targeted Case Manager to join our team. This role is crucial in facilitating comprehensive addiction treatment programs tailored for clients struggling with opioid use disorder and other substances. Our ideal candidate will have a passion for recovery and wellness, acting as a key facilitator between patients, families, and the treatment services provided at BayMark. Develop and manage comprehensive treatment plans in collaboration with healthcare providers to ensure the delivery of quality treatment. Conduct patient assessments to determine appropriate treatment options and to guide clients through the recovery process. Coordinate and monitor the services across various elements of the healthcare and social services spectrum tailored to each client. Provide education and resources to patients and families regarding treatment options and community supports. Regularly evaluate and document patient progress, adapting treatment plans as necessary. Comply with all legal, ethical, and professional standards. Communicate effectively with a multidisciplinary team to enhance the delivery of patient-centered care. In-depth understanding of chemical dependency, addiction treatment modalities, and mental health disorders. Excellent communication and interpersonal skills. Ability to manage multiple cases effectively with strong organizational skills. Proficiency in maintaining accurate and detailed reports and treatment plans.
