This role blends IT insighth with healthcare expertise to optimize patient case management systems, facilitating improved patient outcomes and ensuring regulatory compliance. The ideal candidate will leverage technology to effectively support community health initiatives, manage patient data securely, and enhance interaction between healthcare providers and patients. Assess and analyze patient needs and healthcare outcomes using advanced IT tools. Develop effective case management plans to facilitate comprehensive patient care. Collaborate with healthcare professionals to integrate technology solutions that improve patient tracking and reporting systems. Monitor and report on patient progress, utilizing software to ensure accurate data collection and secure communications. Manage resources efficiently to ensure that patient care is effective, timely, and within budget. Maintain up-to-date knowledge of healthcare regulations and IT advancements to ensure compliance and maximize efficiency. Strong proficiency in database management and understanding of medical software. Excellent communication and interpersonal skills, with an ability to interact at all levels of the healthcare and IT sectors. Critical thinking and problem-solving skills in a high-pressure environment. Ability to handle confidential information with discretion and integrity.
