Compensation: $20.00/hourly

Care Coordinators provide support to individuals experiencing serious mental illness (SMI) who are receiving services from the Forensic Assertive Community Treatment Team (FACT). As a FACT team member, the care coordinator works with community resources who provide services, including shelter, housing, detoxification, mainstream benefits, employment, and transportation. Assists individuals in daily living activities and establishing and maintaining linkages that provide community education, support, and independence. Provide case management services for participants, including assisting participants in obtaining housing, coordinating with criminal justice agencies, and assisting clients with activities of daily living such as medication management and independent living skills. Facilitate recovery planning for participants. On-call rotation: Respond to crisis calls for assessment and pertinent intervention. Report program activities and development, crises, and any other pertinent information to the appropriate chain of command. Be available for 24-7 crisis intervention. Provides services within the requirements for the FACT team Participates as an active member of all team meetings. Proactively involve and communicate with collateral supports, including family, significant others, probation officers, judges, caseworkers, and others. Document all incidents immediately and report as required. Complete accurate and thorough client reports in required timeframes, such as court reports. Other duties as assigned. Requirements
Requirements Licensure / Education / Prior Experience Required: Bachelor's degree in psychology, social work, or related field (preferred, not required). Two years or more of related employment as a care coordinator or interfacing with the community resources that support reentry into the community. (Housing, shelter, employment, etc.) or program graduate recommended by staff for employment that is in pursuit of licensure. Lived experience preferred. Proficient in MS Office applications Trauma Informed Care experience is preferred. Willingness and ability to pass a criminal background check Must be at least 21 years old First Aid and CPR Certification (available at Odyssey House) Completion of Crisis Prevention Intervention (CPI) and Therapeutic Options Training May be required to pass pre-placement physical Must comply with annual TB tests, Flu and Covid immunizations as required Must meet employment eligibility Need to be able to be cleared to drive agency vehicles
Employment offers are contingent upon successful completion of required pre-employment screenings , which may include background checks, fingerprinting, applicable sex offender registry screenings for Adult Residential roles, and other position-related verifications. Roles requiring driving are subject to a Motor Vehicle Record (MVR) review and valid Utah driver's license.

Physical Demands of the Job: Ability to lift up to 15 lbs., drive (or adequate alternate transportation), sit for prolonged periods, and perform light to moderate physical activity at times.
