Role OverviewThe Behavioral Health Coordinator is responsible for acting as a liaison and support person between the Division of Medicaid and Medical Assistance (DMMA) and the Case Management team, the Division of Substance Abuse and Mental Health (DSAMH), and Behavioral Health providers. The Coordinator will also be responsible for outreach activities with members and providers and documenting all activities in the electronic case management documentation system.Work ArrangementThis is a remote field-based position requiring travel within Delaware, mainly Kent and Sussex Counties.ResponsibilitiesCoordinating Member care needs with BH ProvidersDeveloping processes to coordinate behavioral healthcare between Primary Care Physicians and Behavioral Health ProvidersParticipating in identifying best practices for BH in a primary care settingCoordinating behavioral care with medically necessary servicesBeing knowledgeable of the PROMISE requirements and coordinating with PROMISE providers to effectuate the requirementsKnowledge of and experience with behavioral health and substance use disorder providersDevelop and maintain relationships with behavioral health and substance use disorder providersEducation/Experience: Master’s Degree required3-5 yearsprior experience in behavioral health/human services required; or other equivalent background and experience that would translate well to this positionLicensureActive Delaware Licensed Clinical Social Worker (LCSW) requiredSkills & AbilitiesProficiency with Microsoft Office Suite (Word, Excel, PowerPoint); MS Access is a plusAbility to travel up to 25% throughout the state of DEValid driver’s licenseYour career starts now.
