About the Role: We are seeking a passionate and skilled Physical Therapist (PT) to join our growing team. You will provide essential physical therapy services to adult patients at their homes. You will play a vital role in helping individuals recover from injuries, manage chronic conditions, improve mobility, and enhance their overall quality of life. Responsibilities: Conduct comprehensive assessments to evaluate patients' physical condition, functional limitations, and movement impairments. Develop and implement individualized treatment plans using evidence-based practices and therapeutic techniques. Provide direct therapy to patients, addressing a variety of musculoskeletal conditions, neurological disorders, and post-surgical rehabilitation needs. Educate and engage patients and their families in the treatment process, providing them with strategies and resources to support their ongoing progress. Monitor patient progress, documenting observations and making necessary adjustments to treatment plans. Collaborate effectively with Speech and Occupational Therapists to deliver integrated and holistic care. Maintain accurate and timely documentation of patient progress and treatment interventions. Qualifications: Graduate of an accredited Physical Therapy program. Valid and current state license as a Physical Therapist. Demonstrated ability to assess and treat a wide range of adult physical therapy needs. Excellent communication, interpersonal, and patient engagement skills. Experience working with diverse adult populations, including seniors. Knowledge of common adult health conditions and appropriate assessment tools. Experience in home care is preferred.
